The correct form that will need to be completed is based on the employer's location (not the applicant's). For example, if the applicant lives in Ohio but has gained employment in Michigan, they will need to complete the Michigan application.
For MICHIGAN PERMITS:
Step 1, Complete the first section by minor applicant or guardian
Step 2, the employer completes their portion
Step 3, The applicant must physically come in and show proof of age (eligible forms of proof are listed on the form) We can complete the application in the HS main office and give them the permit
Step 4, Return the permit to the employer for their records.
For OHIO PERMITS:
Ohio's application process is similar, with an additional step to have the back portion completed by their doctor, or attach a physical form that has been dated within the past 12 months (student athletes usually have these forms uploaded in final forms and can print off). They would then need an Ohio school (We frequently contact Whitmer. We can scan over the form after verifying their identity and age, or they can stop in their office themselves) to complete the process to obtain a work permit.
